This includes but is not limited to the day-to-day management and supervision of the related clinical activities within the area on the nurse's tour of duty.
The Primary Nurse will assume responsibility for implanting the Plan of Care for patients assigned which includes but is not limited to:
Coordinating with the Case Manager appropriate changes in the Plan of Care with regards to care provided, frequency, and referral to other services.
The nurse is responsible for supervising and responsibly directing the activities of various levels of assigned nursing and ancillary personnel utilizing both professional and supervisory discretion and independent judgment.
Essential Functions Employee must have regular attendance/punctuality, be able to work with others at all levels of the Company, have exceptional customer service, and be completely honest.
Other assigned duties include:
Provides direct patient care as described in state regulations for LPN/LVN, in all cases involving nursing services.
Assists in the implementation of changes in the Plan of Care for the improvement of client care.
Makes the initial evaluation visit as applicable and re-evaluates the patient's needs on a regular basis.
Provides nursing services, treatments and diagnostic procedures requiring specialized skills and assuming full responsibility for each assigned patient.
Participates in in-service programs, as well as training, and teaching other nursing personnel as required.
Assists client in learning appropriate self-care techniques.
Promotes and maintains client health through teaching and appropriate rehabilitative measures.
Develops and implements the treatment plan for each patient under the direction of their physician and plan of care.
Observes and reports symptoms, reactions to treatments, drugs, and changes in the patient's physical and/or emotional condition.
Initiates preventative and rehabilitative nursing procedures as appropriate for the patient's care and safety.
Maintains clinical and progress notes for each assigned patient receiving care and providing progress reports to their physician.
Coordinates services for patients, as needed, to other agencies and health care providers as appropriate.
Counsels the patient and their family in meeting nursing and related needs, Provides supervision of Home Health Aide/Home Care Aide/Certified Nursing Assistant services and prepares written instructions for care provided by HHA/HCA/CNA.
Demonstrates self-direction and ability to work without the presence of immediate supervisor.
Considers age-related factors with patient care for the adult and elderly when providing home care.
Maintains accurate time records.
Submits required documentation in a timely manner.
Reports observed or suspected child or adult abuse pursuant to mandated requirements.
Assists State Nurse Administrator, Clinical Supervisor, and Clinical Manager in decision making and policy formation as it pertains to the Home Health Program.
Participates in surveys, studies and special projects as assigned.
Able to see and hear adequately to respond to auditory and visual requests.
Demonstrates a strong sense of initiative and urgency in addressing unexpected patient crisis.
Demonstrates knowledge regarding exposure to body fluids and chemicals, as well as some hazardous equipment.
Other duties as assigned.
SUPERVISORY RESPONSIBILITIES This position has indirect supervisory responsibilities for Aides.
This position will perform supervisory visits as needed Requirements Related Competencies Language Skills:
Must be able to read, analyze, and interpret general business practices, policies and procedures, professional journals, technical guidelines, and governmental regulations.
Must be able to write reports, business correspondence, and procedure manuals.
Must be able to effectively present information and respond to questions from groups of managers, clients, customers, and the public.
Must be able to provide guidance and direction accurately and clearly to Company staff and employees.

Other Requirements:
Maintain current state licensure.
Maintain current liability and malpractice insurance, if applicable.
Education/Training Licensed Practical Nurse/Licensed Vocational Nurse license to practice professional nursing in each state where patient care is supervised, current Basic Cardiac Life Support certification.
Licensure/Certification Valid Driver's License Valid and current CPR Certification Valid and current License Practical Nurse license in the state of practice Clearances The following background checks are conducted:

The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
